Position: Patient Care Coordinator

Your Mission:
As our Patient Care Coordinator, you will play a vital role in ensuring smooth, efficient care for our patients. Your responsibilities include:

• Educating patients on care plans, treatments, testing, supplements, and continuing care requirements.
• Serve as the primary point of contact for patients, addressing inquiries and providing information regarding services offered.
• Maintaining quality service by establishing and enforcing organizational standards.
• Managing and updating patient care databases.
• Coordinating interdisciplinary management tools and evaluating care interventions with the team.
• Contributing to team success by accomplishing related tasks as needed.
• Collaborate with medical staff to coordinate patient care and facilitate communication between patients and providers.

Qualifications:

• Familiarity with holistic and integrative medicine (preferred)
• Experience with Women's Health
• Proficient in EMR software
• Strong communication and interpersonal skills
• Self-starter, must be willing to adapt to meet the patients' needs
• Ability to multitask and work in a fast-paced environment
• A heart full of empathy and a genuine love for caring for others
• Excellent organizational skills with the ability to multitask in a fast-paced environment.
• Strong interpersonal skills with a focus on providing exceptional patient care.
• Ability to work collaboratively as part of a team while also being self-motivated to complete tasks independently.
