Position Summary
The Lead Care Manager oversees Community Health Workers (CHWs) and Care Coordinators within designated program areas, ensuring services meet program goals, managed care plan contracts, and DHCS requirements. This role combines direct care management for vulnerable populations with team leadership, program oversight, and stakeholder engagement.
Key Responsibilities
Care Management
- Conduct psychosocial and health assessments, develop individualized care plans, and monitor progress.
- Coordinate care across medical, behavioral, and social service systems.
- Link patients to community resources addressing social determinants of health.
- Provide patient/family education and engagement using evidence-based approaches.
- Facilitate Interdisciplinary Care Team (IDT) meetings and maintain accurate documentation.
Leadership & Supervision
- Oversee patient caseloads and provide clinical consults to non-clinical staff.
- Supervise, train, and mentor CHWs and Care Coordinators.
- Support quality improvement initiatives and program compliance.
Program Oversight
- Ensure effective delivery of Enhanced Care Management (ECM) services.
- Track performance metrics and use data to refine program goals.
- Implement tools and processes that meet managed care and regulatory requirements.
Community & Stakeholder Engagement
- Build and maintain partnerships with community organizations, funders, and stakeholders.
- Represent the organization in community settings to advance mission awareness.
- Develop internship opportunities in collaboration with local colleges.
Qualifications
- Master’s degree in Social Work (MSW) and current California LCSW license.
- Experience in care management, healthcare, or related settings.
- Strong background working with vulnerable populations and chronic conditions.
- Proven leadership and supervisory skills.
- Excellent communication, collaboration, and problem-solving abilities.
- Familiarity with California nonprofit compliance and managed care programs preferred.

Job Type: Full-time
Pay: $31.25 - $35.00 per hour
Expected hours: 40 per week
