St. Mary’s Home Health is seeking Part-Time, Per Diem Medical Social Worker.

In this position you will:

• Assess the psychosocial status of patients related to the patient's illness and environment.
• Carry out social evaluations and plan intervention based on evaluation findings.
• Maintain clinical records on all patients referred to social work.
• Provide information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
• Provide information to patients or families/caregivers and community agencies.
• Serve as liaison between patients or families/caregivers and community agencies.
• Maintain and develop contracts with public and private agencies as resources for patient and organization personnel.
• Participate in the development of the total plan of care and case conferences as required.
• Assist physician and other team members in understanding significant social and emotional factors related to health problems.
• Participate in discharge planning.
• Other duties as delegated by the Clinical Director/Supervisor.

Qualifications:

Required qualifications for this position include:

• Graduate of a Master's program in social work accredited by Council on Social Work Education
• Minimum of 1 year experience in health care
• Demonstrates good verbal and written communication, and organization skills.
• Current California Driver’s License and Automobile Insurance

Preferred qualifications for this position include:

• Hospice, home health, long-term care, medical/surgical, and/or oncology experience
• Knowledge of Home Health practices
• Knowledge of Electronic Health Record systems
