Role & Responsibilities

As our MSW Supervisor (Permanent), you’ll play a key role in supporting our team, participants, and community:

• Provide oversight and guidance to social work staff in line with PACE program goals.
• Conduct assessments and ensure care plans meet the psychosocial needs of frail and/or elderly participants.
• Perform counseling using effective theories and techniques—because life doesn’t come with a manual, but you know how to help.
• Collaborate with interdisciplinary teams to ensure holistic care.
• Complete and maintain documentation in EPIC EMR and other systems efficiently.
• Travel across San Diego county for participant home visits and assessments (don’t worry—we’ll keep the coffee flowing).
• Connect clients with community resources and support services.
• Lead with empathy, humor, and strong project management skills.

What You Bring

• Education: Master’s Degree from an accredited School of Social Work.
• Experience: 1+ year in a healthcare setting, preferably with elderly or frail populations.
• Knowledge & Skills:
• Counseling theories and techniques.
• Proficiency with EPIC EMR.
• Strong organizational and interpersonal skills.
• Ability to work independently and meet deadlines.
• Certificates/Licenses:
• Valid California driver’s license & reliable transportation (must meet PACE guidelines).
• Current First Aid/CPR certification.
