The Care Coordinator works with the Intensive Behavioral Health Program.

This position is hybrid/ remote with at least one day in the office weekly and schools as needed.

Description:

The IBHS Care Coordinator is the person who is required to ensure that all the required paperwork for specified service authorizations is submitted to the insurance agencies in a timely fashion. The Care Coordinator is in collaboration with all treatment team members. This position ensures all needed reports and documents are procured from appropriate staff, and paperwork is submitted within the appropriate time frame. The Care Coordinator is responsible for notifying all parties needed when authorization is received. This ensures there is no interruption in the services provided to the client and their family. This includes, but not limited to: the completion of “the funding packet”. Appropriate response formulation for insufficient information that has been requested and timely staff notification. The care coordinator will have weekly supervision with the director and/or designee.

Responsibilities:

· Establish and/or maintain a census report with the client expiration dates. Staff's list addendums, and memos necessary to ensure time restraints for authorization.

· Coordinate with treatment team and staff as required to ensure continuity of care.

· Coordinate all information and required documents needed to the Behavioral Consultants to ensure timely submittal. This would also include making sure the Behavior consultant is aware of Team meetings.

· Ensure census report is updated of evaluation appointment and meetings.

· Timely notification to all team members of change in authorization status and/or reauthorization, with verification of the time frames for authorization.

· Notification to parents and team members of eligibility issues. Continuously following up with eligibility issues and cease case services until the eligibility problem is resolved.

· Monthly record review to ensure quality assurance.

· Coordinate with the IBHS Director and/or Assistant Director regarding any issues or concerns and all seek guidance when needed.

· Complete authorization paperwork within the identified time frames to ensure continuity of service and quality of care.

· Ensure timely response from regarding any corrections needed for evaluations and insufficiencies.

· Update authorizations

· Act as a liaison between families and team members. This would include which providing support for the Client with any questions or issues they might have.

· Attain proficiency in electronic health records programs and software and any other needed electronic support programs.

· Other duties as assigned.

Qualifications

· Education-minimum of an associate degree.

· Ability to understand mental health services and seek guidance when in question.

· Ability to work cooperatively and professionally.

· Ability to represent the agency in a professional and competent manner.

· Ability to compose paperwork in organized, concise, and timely manner.

· Mindset of being a Team player

· Excellent communication skills, this includes any type of communication used.

· Organization skills is a must, and being able to recognize and prioritize duties
