Job Summary:
The Personal are Coordinator provides specialized services for assigned participants. CNA is preferred, but not required.
This is a non-exempt position.
Pay rate is $20.34 per hour
If you have an active CNA license pay rate is $21.13 per hour
Supervisory Responsibilities: None

Essential Functions of Position:

  • Works cooperatively with staff and program participants as assigned to implement objectives and therapeutic activities that support Service Plan goals. Ensures that the participants’ needs are met and communicated with program staff and treatment team according to regulations and prescribed need.
  • Coordinates participant’s personal care schedule with program staff to meet all the needs of the program. Performs personal care functions for individuals with disabilities, including changing, toileting, feeding, and laundry. Uses adaptive equipment and mechanical lift systems competently as per training and supervision to facilitate the movement and lifting of physically disabled individuals for personal care assistance.

  • Trains participants to maximize independent functioning in self-care and activities of daily living related to personal care, including personal hygiene, grooming, and eating. Participates in the development and implementation of Day Habilitation Service Plans and IDT as a member of the Day Habilitation team.
  • Reports medical and health concerns to the Healthcare Supervisor and Program Director. Provides emergency interventions as needed.
  • Delivers services to program participants with respect and dignity in accordance with human rights training, DDS regulations for human rights, and Bay Cove standards.
  • Provides participant coverage for other groups and activities as assigned.
  • Demonstrates proficiency with the use of electronic database, email, electronic record keeping and on-line training systems. Provides required documentation for participant activities and personal care records for each day.
  • Other duties as assigned by the Program Director, Day Hab Service Manager or R.N.
Requirements for the position:
  • High School degree
  • Certification as Nurse’s Aide is required.
  • One year of job experience in a training program working with this population and/or Associate Degree in human services related field preferred
  • Maintain required training certifications including CNA, CPR, and First Aid.
  • Must have had a TB screening within the past 12 months, & required every year.
  • Ability to lift 30 pounds. Ability and physical capacity to perform physical care tasks to assist people with mobility impairments as needed, utilizing safety protocols and approved techniques.
