Organizational Overview: Public Guardian Services (PGS) is a privately funded, non-profit organization, offering low- or no-cost guardianship and conservatorship services to indigent, isolated persons in Massachusetts who need medical, personal or financial decisional support. PGS is directed by an Executive Director and operates in Suffolk, Plymouth and Norfolk Counties.

Position Summary: The Guardianship Care Manager will oversee and coordinate legal, medical, financial, social, and psychiatric case management services, fulfilling the role and duties of Guardian for diverse populations of individuals who have been declared by the Probate and Family Court to lack capacity.

The Guardianship Case Manager provides comprehensive assessments and monitoring to ensure the effective delivery of services and coordinates all available resources for clients for whom the agency is guardian. Utilizing good judgment and communication skills, the Guardianship Care Manager will work closely with other PGS staff, the Client, Court, family members, health care providers, and with various social service agencies as needed. A clinical tool entitled the Guardianship Acuity Initial Screen (GAIS), will be implemented by the Chief Clinical Officer to ensure manageable caseload mix for all PGS Care Managers.

Essential Job Functions:

  • Manage client caseload of 20 individuals
  • Develop a relationship with client to the extent possible
  • Visit each client at least monthly
  • Observe and evaluate client’s care and well-being (e.g. nutrition, sleep, medications, clothing, medical care, physical/mental)
  • Maintain appropriate communication with Client between visits.
  • Initiate or respond to communication as needed with support networks or treatment teams
  • Act in the best interests of client
  • Oversee decision-making for client to maximize autonomy and independence
  • Secure all financial resources available to client
  • Keep reliable and up-to-date client records, using PGS Manager
  • Complete, sign, serve and file statutory reporting requirements in a timely manner for each appointment as Guardian, Representative Payee or Conservator
  • Provide after-hours coverage for emergencies, as part of the team of PGS Care Managers, on a rotating basis.
  • Actively participate in regularly scheduled one-on-one supervision
  • Effectively contribute to the organizational life of PGS through participation in team meetings, peer collaboration, program development and/or research.
  • Performs other duties and responsibilities as assigned.

Education/Qualifications:

  • Master’s degree in Social Work or similar human services field preferred, and experience with elders, people with ID/DD or mental health disorders. Licensure preferred.
  • Ability to face adversity, unpopular stands, or engage in tough debates when necessary.
  • Excellent written and oral communication skills.
  • Advocate for clients when needed.
  • Proficient/skilled in Microsoft Office applications.
  • Driver’s License and car required.
  • Can perform job duties in a stressful environment.
  • Experience in guardianship preferred.

Job Type: Full-time

Pay: $70,000.00 per year
