Job Summary

Responsible for managing a client caseload including screening and assessing for client needs, development of treatment

plans, facilitating individual and group counseling sessions, documentation of client progress and participation, treatment

plans reviews and discharging planning. Knowledge of substance abuse 12 core functions and able to perform and apply

these as such. Works directly under the supervision of an LCDC-QCC.

Essential Duties And Responsibilities

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Conducts orientation to program and develops psychosocial history and treatment plans for clients within

time frame specified in program level of service.

  • Maintains client files and documentation in accordance with agency, state, and federal rules.
  • Facilitates psycho educational individual, group, and family sessions to include alcohol and drug education, as

assigned by Program Director.

  • Participate in consultation with treatment team and other appropriate individuals or programs to accomplish

treatment goals.

  • Remains current with community resources and completes all required and relevant documents, including

documentation of client services.

  • Attends agencies trainings and continues to acquire proper Continuing Education (CE) hours.
  • Provide required documentation to utilization reviewers for coordination of client care.
  • Responsible for completing all discharge paperwork within time frame established by agency policy and

submitting to appropriate person/department and conduct follow-ups after sixty days of discharge.

  • Develops and maintains a positive, professional working relationship with the staff, clients and community;

Complies with Aliviane Inc. Confidentiality Policy assuring that agency/program/employee records/information

is protected at all times.

  • Understands, accepts, implements and promotes Aliviane Inc. Mission Statement, program/department goals,

philosophy and objectives; Serves as a role model for employees, clients and community partners. Performs other duties as assigned.
