The Care Coordinator in the Interventional Cardiology Clinic is responsible for streamlining patient care by working closely with multidisciplinary teams, triaging referrals, scheduling providers, and ensuring continuity across outpatient and inpatient settings.

• Acts as a liaison between referring providers, the interventional team, and patients to facilitate seamless communication and care.
• Supports program growth through outreach and effective data management.
• Location primarily in Texas and surrounding areas.
• Requires RN licensure in Texas, with a BSN preferred.
• Three years of recent hospital experience, preferably in cardiovascular or interventional cardiology nursing, is preferred.
• Knowledge of cardiac procedures, catheterization lab operations, and clinical workflows is essential.
• Experience with Epic or other EHR systems, and patient navigation in cardiology, is advantageous.
• Must hold current AHA BLS certification.
