Description:

We are seeking a compassionate and dedicated Home Health and Hospice Social Worker to provide comprehensive psychosocial support to patients and their families in their homes. The ideal candidate will possess strong assessment, intervention, and communication skills, with a deep understanding of the emotional, social, and practical challenges associated with serious illness and end-of-life care. This role involves collaborating with an interdisciplinary team to ensure holistic care, advocate for patient needs, and connect families with appropriate community resources.

Responsibilities

  • Conduct initial and ongoing psychosocial assessments of patients and families to identify emotional, social, environmental, and financial needs.
  • Develop and implement individualized care plans based on assessment findings, addressing grief, loss, coping strategies, and crisis intervention as needed.
  • Provide counseling and emotional support to patients and families navigating illness progression, end-of-life decisions, and bereavement.
  • Educate patients and families on available community resources, support groups, and advance care planning options.
  • Facilitate communication between patients, families, and the interdisciplinary team to ensure a coordinated approach to care.
  • Act as a patient advocate, ensuring their rights and preferences are respected.
  • Document all patient interactions and interventions accurately and timely in accordance with agency policies and regulatory requirements.
  • Participate in interdisciplinary team meetings to discuss patient progress, revise care plans, and ensure continuity of care.
  • Provide bereavement support to families following a patient's death.
  • Adhere to all ethical guidelines and professional standards of social work practice.
  • Maintain confidentiality of patient information.
Requirements:

Qualifications

  • Master's degree in Social Work (MSW) from an accredited school of social work.
  • No experience needed, but experience with elderly patients preferred
  • Knowledge of psychosocial aspects of illness, grief, loss, and end-of-life care.
  • Strong assessment, crisis intervention, and counseling skills.
  • Excellent written and verbal communication skills.
  • Ability to work independently and as part of an interdisciplinary team. Demonstrated empathy, compassion, and cultural sensitivity.
  • Proficiency in electronic health record (EHR) systems.
  • Valid driver's license and reliable transportation.

Preferred Qualifications

  • Experience with specific patient populations (e.g., geriatrics, oncology).
  • Certification in Hospice and Palliative Social Work (e.g., APHSW-C).
