Social Worker
Location: Nazareth Hospital

Schedule: Full-Time | Includes Weekend Rotations
Position Description
Nazareth Hospital is seeking a compassionate and experienced Social Worker to join our Care Coordination team. Reporting to the Manager of Care Coordination, this role is directly involved in the assessment, planning, facilitation, and advocacy of services to meet individual patient healthcare needs. Through effective communication and resource identification, the Social Worker promotes quality outcomes, patient satisfaction, and cost-effective care delivery.
What You Will Do
+ Collaborate with Care Coordination staff to assess patient needs and develop individualized care plans.
+ Facilitate access to community resources, entitlements, and support services.
+ Advocate for patients and families to ensure continuity of care and appropriate discharge planning.
+ Participate in interdisciplinary team meetings to support care transitions.
+ Utilize information systems (including MIDAS) to document assessments and interventions.
+ Build and maintain strong relationships with patients, families, and healthcare providers.
+ Support patients in navigating managed care, government payers, and third-party reimbursement processes.
Minimum Qualifications
+ Education:
+ Master’s Degree in Social Work (MSW) required.
+ Licensure/Certification:
+ Licensed Social Worker (LSW) in the State of Pennsylvania preferred.
+ Experience:
+ Minimum of 3 years of experience in a hospital, home health, managed care, or other health-related setting preferred.
+ Skills:
+ Knowledge of community resources and entitlement programs.
+ Familiarity with managed care and reimbursement systems.
+ Proficiency in Microsoft Office and clinical documentation systems (MIDAS preferred).
+ Strong interpersonal and communication skills.
