Cuda Counseling, LCSW PLLC is a private practice of experienced licensed mental health professionals currently serving more than 1,000 clients in Herkimer and Oneida County. Our goal is to continue to serve a broad range of patients across all ages, including children, adolescents, adults and families, with issues ranging from anxiety, depression to mood disorders and substance abuse. We utilize multiple evidence-based modalities including, but not limited to, cognitive behavioral therapy, dialectal behavioral therapy, play-therapy for children and solution focused treatment.

Job Description

Job Title: Outpatient Mental Health Therapist

Reports To: Owner or Clinical Supervisor

Summary:

Responsible for planning and carrying out psychiatric mental health and alcohol and drug abuse treatment services to consumers. Assess, diagnose, treat, counsel and refer consumer of the organization. Provide crisis intervention services. Provide psychosocial evaluations and assessments.

Duties and Responsibilities include the following. Other duties may be assigned.

1. Provide initial evaluation and assessment interview upon consumer’s admission. Make recommendation for treatment and referral as appropriate.*

2. Develop individualized treatment plans, defining plan of services and intervention. Update treatment plans according to documentation requirements.*

3. Provide individual, couple, group and family therapy according to the consumer’s treatment needs. Provide crisis intervention services as needed.*

4. Document clinical service delivery within 72 hours of consumer contact. Documentation meets all appropriate HIPPA and Medicaid/Medicare documentation guidelines.*

5. Keep records of consumer contacts as specific by accounting and statistical needs of the organization. Complete all required OTR’s/PA’s to allow for billing of clinical service in a timely manner.*

6. Maintain appropriate size caseload in meeting productivity expectations.*

7. Attend outpatient staff meetings for agency communication, clinical consultation and trainings.*

8. Coordinate provision of clinical services for consumers with other programs and/or medical services.*

9. Abide by all professional, certification, and corporate ethical standards in performance of duties. Maintain current licensure to be in good standing with the licensure board.*

10. Other duties as assigned.*

Qualifications:

Language Ability:

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Math Ability:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills:

To perform this job successfully, an individual should have knowledge of Word Processing Software and Medical Recordkeeping System.

Education/Experience:

· Master's degree (M. A. / M. S.) or equivalent;

Certificates and Licenses:

· Current licensure in the state of New York
