Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to the Maternal Child Health Supportive Services Team. The Reproductive and Child Health Care Coordinator role requires someone who enjoys working independently and as a part of a team to support the needs of the patients, providers, care team and health center site. This position is responsible for coordinating and monitoring the Reproductive Health Providers Team’s assignments and deliveries, as well as enrolling and assigning patients in case management. The RCHCC will also provide education to prenatal patients and families with infants up to age one, conduct patient outreach, enter data, and run reports as well as facilitating referrals and follow-ups when needed.

- Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
Main Duties & Responsibilities
- Provides early pregnancy teaching/education, family planning education (option counseling, parenting, termination, and adoption), and a third trimester packet.
- Conducts the verification of pregnancy visit. Completes the verification of pregnancy letters and refers patients for Kid Care, MPE, WIC, SDOH screenings, case management services, etc. as appropriate.
- Schedules new OB appointments with appropriate provider (CNM or MD). Assigns and coordinates the expected deliveries among the assigned site Reproductive Health Team.
- Documents patients care and care coordination activities in appropriate electronic system (s) (EMR, Vp Tracker, State Case Management database) and maintains accurate documentation and data entry for timely report production and distribution including the maintenance of the site EDC list and any OB statistics.
- Coordinates and maintains a log of contraceptive choices such as LARCS/Intrauterine device (IUD) orders and distribution. (Based on site clinic flow)
- Support with partner hospital and grant related data reporting needs as requested.
Education
- High school diploma or equivalent required
- (2) Two years of relevant healthcare or community experience required
- (2) Two years working with patients of reproductive health, including pregnancy, childbirth, post-partum, and gynecological issues required
Skills and Knowledge
Required:
- Active CPR Certification required
- Cultural competency to work with a diverse patient population
- Strong computer and data skills
- Attention to detail
- Interpersonal and oral communication skills
- Comfort with using telephone interpretation services required
Preferred:
- Associates degree or (2) years of college is a plus
- Bilingual fluency (English/Spanish) preferred. (May be required to assist or work with non-English/Spanish only speaking patients and/or guests).
