Job Overview

Key Responsibilities

• Lead the delivery of in-home physical therapy services.
• Evaluate function and mobility using objective measures.
• Collaborate with patients, families, and physicians.
• Provide caregiver training and support.
• Monitor progress and adjust therapy accordingly.
• Maintain detailed clinical records.

Requirements

• Current PT license.
• One year of professional experience.
• BLS certification.
• Reliable transportation.

Preferred Qualifications

• Advanced degree (DPT).
• Prior home health experience.
• Certification in OASIS.
