LMSW - Social Worker - Temp/Contract - 3 months!

***MUST HAVE VALID NY STATE LMSW LICENSE and valid Drivers License

***Comfortable doing home visits/field work

Job Responsibilities:

• LMSW Conduct assessments to determine client needs and develop appropriate intervention plans.
• Provide ongoing casework services to members and their authorized representatives to help them understand accept and follow care recommendations, to assist them with personal and environmental difficulties.
• Social Worker will develop discharge plans and ensure that the services the member requires are in place in order to facilitate a timely and safe discharge and ensure that the members will be cared for at home.
• LMSW will advocate for clients' rights and access to necessary support systems
• Social Worker will provide crisis intervention and management as needed
• Conduct home visits to assess living conditions and ensure the safety and well-being of clients.
• Provide counseling and support to individuals dealing with behavioral health issues.
• Develop and implement workforce development programs to help clients gain employment skills

Education & Certification:

• Required Master's degree in Social Work from accredited college or university
• At least 1+ years of experience in a healthcare setting - preferably with geriatrics
• Must have a car for field visits - field experience preferred
• CPR/BLS
• Current LMSW licensure from New York State

Pay: $40.00 - $45.00 per hour

Expected hours: 36 – 40 per week
