Job Title: Care Manager (Full-time or Part-time)

Organization: Community-Based Service Organization

Job Summary:

We are seeking compassionate and organized Care Managers to join our team. In this role, you will be instrumental in supporting the mental health and well-being of our clients. This position offers an excellent opportunity to gain experience in the mental health field, enhance your skills, and advance your career.

Requirements:

  • Bachelor’s degree or college credits in a related field
  • Strong organizational skills with the ability to work both independently and collaboratively
  • Bilingual in Spanish preferred
  • Work in the office. Should be able to travel Bronx to meet clients
  • CANS certified

Responsibilities:

  • Conduct assessments
  • Meet with clients in the field once a month (Bronx clients)
  • Develop care plans to address gaps in clients' care
  • Maintain ongoing relationships with clients to promote their health and well-being
  • Connect clients with resources and collaborate with their providers and family
