FellowshipLIFE is searching for an exceptional part time Social Worker/ Volunteer Coordinator with NJ LSW or LCSW who is passionate and understanding of hospice philosophy and principles of death and dying. Responsibilities:

• Plan, coordinate, manage and implement support packages to help patients deal with difficulties during the end-of-life transition.
• Develop, prepare, and maintain clinical documentation with accuracy, timelines and according to prescribed policies.
• Monitor and evaluate patients’ progress and modify care plans accordingly.
• Advocate for patients and their families and assist them in obtaining appropriate care and access resources aimed at increasing their quality of life.
• Participates with the interdisciplinary team.
• Coordinating hospice volunteer services, including recruiting, training, assigning/ scheduling, supervising and retaining volunteers.
• Planning and implementing annual volunteer program/ event.
• Monitor volunteer activity and patient/ family satisfaction.

Requirements:

• Master’s degree in social work
• NJ LSW or LCSW
• Understanding of or willingness to learn hospice philosophy and principles of death and dying.
• 3+ years’ experience in a healthcare setting, community health experience preferred.
• Hospice Experience preferred
• Excellent verbal and written communication skills
• Ability to effectively interact with a diverse population.

• Capable of working independently and collaboratively in a multidisciplinary team
• Proficient in electronic medical record systems and relevant software applications
