Primary Responsibilities and Essential Functions
➢ Conducting care-planning assessments to identify preferences and needs
➢ Screening, arranging, and monitoring cost-effective resources and services
➢ Coordinating with other care providers to ensure continuity of care
➢ Reviewing financial, legal, or medical issues
➢ Offering referrals to specialists to address identified needs
➢ Providing crisis intervention
➢ Acting as a liaison to other service providers, healthcare professionals, and families
➢ Providing client, family, and community education and advocacy
➢ Offering compassion and support during times of change and/or uncertainty
Ability to
➢ Communicate effectively orally and in writing

➢ Actively participate and facilitate meetings

Minimum Qualifications
A bachelor’s degree in social work, gerontology, nursing, or PT, OT, or SLP is required. Two years of care management or relevant experience is preferred.
