Position Overview
The Care Coordinator is a dynamic, client-facing role that blends scheduling, service oversight, and EVV compliance. This position is ideal for professionals with experience as a Scheduler, Client Services Supervisor, or EVV Specialist in home care or home health settings. You’ll be the operational heartbeat of our Scranton or Reading office—ensuring caregivers are supported, clients receive consistent care, and systems run smoothly.
Key Responsibilities
Coordinate caregiver schedules to ensure consistent, high-quality coverage
Respond to call-outs, cancellations, and urgent staffing needs with speed and empathy
Serve as a primary point of contact for clients and caregivers, addressing service inquiries, concerns, and requests with professionalism and care
Answer and triage a high volume of incoming phone calls from clients, caregivers, and referral sources
Provide timely, courteous, and solution-oriented support to callers, escalating issues when necessary
Communicate clearly with clients, caregivers, and internal teams regarding service updates and scheduling changes
Train caregivers on EVV systems (e.g., HHAeXchange), including clock-in/out procedures
Monitor EVV compliance and resolve issues related to telephony, mobile app, or paper logs
Maintain accurate documentation in agency software and update client and caregiver records
Support client satisfaction by following up on service quality, resolving complaints, and ensuring prompt issue resolution
Uphold HIPAA standards and ensure confidentiality of all client and caregiver data
Qualifications
High school diploma or GED required
Two or more years of experience in home care, home health, or healthcare scheduling preferred; comparable customer service experience in a fast-paced, service-oriented environment will be considered
Proficiency in Microsoft Office and scheduling platforms (HHAeXchange experience a plus)
Strong organizational and multitasking skills
Excellent communication and interpersonal abilities
Customer service experience in a healthcare or service-oriented setting strongly preferred
Bilingual (English/Spanish) preferred
Familiarity with Medicaid waiver programs and EVV regulations a plus
