Position: Social Care Coordinator (Staten Island)

Summary:
ACCO, a nonprofit CBO, seeks a Social Care Coordinator to connect Medicaid populations in Staten Island to essential social supports. The role involves assessing health-related social needs, confirming eligibility for Social Care Network (SCN) services, and facilitating access to food, housing, and transportation resources using an online referral platform. This position combines fieldwork in Staten Island with office-based tasks and requires close collaboration with clients, community partners, and healthcare providers.
Key Responsibilities:
- Conduct outreach and screen Medicaid populations for social needs.
- Refer clients to appropriate community-based services and follow up to ensure needs are met.
- Build expertise in local services and SCN resources.
- Document activities and manage workflows using SCN’s IT platform.
- Participate in meetings and share best practices.
- Highlight participant success stories and assist with training and reporting.
Qualifications:
- 1-2 years’ experience in care coordination or human services.
- Strong organizational, problem-solving, and communication skills.
- Proficiency in using technology platforms and engaging with diverse populations.
- Proficient in Mandarin and English fluency; Cantonese preferred.
- Bachelor’s degree in community health preferred.
