Job Description:

24/7 Home Care is seeking a Spanish speaking Intake Coordinator to join our team in Brooklyn, NY. This position will be responsible for the intake of new patients, following up with referrals, and other duties as assigned.

Position : Intake Coordinator – Home Care

Intake Coordinator assists new patients with homecare needs as well as accepts new referrals from our partners. The Intake Coordinator must maintain open lines of communication with referral sources, Service Coordination, Operations, and within personnel in the Nursing/Contract departments.

Required Education and Experience:

High School Diploma or GED is required. Experience in a healthcare setting is preferred.

Job Type: Full-time

Pay: $19.00 - $23.00 per hour
