- Completes assessment of housing and housing related needs.
- Works with Venture House and Apartment Treatment staff, and multi-disciplinary treatment team to establish individualized service plans, which meet the clients’ goals.
- Establishes and maintains effective relationships with other service providers: e.g. Madison County DSS, SPOA Coordinator, PROS, Care Coordination, etc.
- Acts as program liaison and primary contact for other service providers as it relates to clients coordination of services.
- Demonstrates current understanding of the Office of Mental Health regulations, mental health disorders and its treatments. Understands reciprocal relationships between substance abuse and mental health.
- Assists individuals with linkage and referral to substance use disorder and mental health services where necessary.
- Responds to and effectively handles emergencies, which may require flexibility of time and area of assignment.
- Completes all required documentation, in timelines in accordance with program standards.
