Crest Manor is seeking a full-time Director of Social Work for our 80-bed nursing facility. Qualifications/Requirements:

• Knowledge of BIMS, PHQ-9 preferred
• Working Knowledge of MDS 3.0, BIMS, PHQ-9 preferred
• Familiar with admission and discharge planning
• Strong understanding of resident rights, care planning, and Federal/State regulations
• Ability to collaborate with interdisciplinary team
• Excellent communication skills
