Position Summary:
Visiting Counselors is seeking compassionate and experienced LCSWs to provide in-home and telehealth counseling services to older adults within Suffolk County. This role is ideal for clinicians who value flexibility and wish to make a meaningful impact by supporting seniors facing emotional, behavioral, and life transition challenges.

Responsibilities:

• Provide individual psychotherapy to older adults in their homes and via telehealth.
• Conduct comprehensive assessments and develop individualized treatment plans.
• Maintain timely and accurate clinical documentation in accordance with agency policies.
• Coordinate care with primary care physicians, family members, and other service providers as appropriate.
• Follow agency guidelines regarding safety, privacy, and ethical standards.

Requirements:

• Active New York State LCSW license in good standing. Consideration given to social workers with their LMSW and extensive experience.
• Experience working with older adult populations preferred.
• Strong clinical skills in areas such as depression, anxiety, grief, and adjustment disorders.
• Reliable transportation and ability to travel within assigned regions (for in-home services).
• Proficiency with telehealth platforms and electronic documentation systems.

Job Type: Part-time

Pay: $36,000.00 - $40,000.00 per year
