GENERAL DESCRIPTION:

The Residential Social Worker, under general supervision of the Program Director, will develop and nurture therapeutic relationship with students and their families; deliver all mandated counseling services as described by a student’s Individualized Education Plan; deliver program driven related services; maintain appropriate communication with parent/legal guardian; evaluate and report information appropriate to treatment plan development and maintenance; where appropriate, participate in linkages between the residence and outside social services; where appropriate, coordinate, secure, evaluate, and report applicable social service information; develops, supports, and maintains linkages between the residence and outside social services.

MAJOR DUTIES AND RESPONSIBILITIES:

  • Provide all required student counseling mandates as indicated on each child's IEP.
  • Maintain all necessary related service counseling attendance cards and log notes as required to meet Medicaid reporting mandates.
  • Maintain compliance with social service requirements of the Office of Children and Family Services (case notes, service plans, family contacts, etc.).
  • Make phone contact with parent/legal guardians when appropriate to therapeutic relationship and professional responsibilities.
  • Participate in referral & support services for students and families where appropriate.
  • Maintain contact with outside social service agencies and mental health professionals.
  • Insure that the school's policies and procedures regarding Child Abuse and Neglect are understood and implemented on behalf of all students at Martin De Porres.
  • Provide input on monitoring students taking psychotropic medication.
  • · Provide feedback to Consulting Psychiatrist for students taking psychotropic medication.
  • Provide staff with ongoing consultation around relevant group and individual behavioral management needs, medication, relevant personal, psychological, and family data that affects school/residential functioning.
  • Provide administrative staff with consultation around relevant system-wide issues in the school and community/family milieu of the students.
  • · Provide updated social histories as required.
  • Generate Behavioral Intervention Plans and Functional Behavioral Analysis Reports as required.
  • Provide annual progress reports and recommendations to the Committee on Special Education.
  • Attend IEP annual review conferences as determined by the Supervising Social Worker in consultation with the Director of Education.
  • Provide administrative support and presence when a student requires emergency assistance (hospital, police, medical support).
  • Provide contact with a student's family as often as required to insure a defined treatment plan is in place for each student.
  • Insure that Transitional Supports are in place when a student leaves the Martin De Porres Residential School.
  • Assist with the Intake Screening process for new students.
  • Provide parent support as determined by the School Principal and at a location mutually agreeable to all parties.
  • And all other duties as assigned by the Martin De Porres School Administration.

  • Social Work Interns

QUALIFICATIONS

EDUCATION:

  • NYS Licensed Clinical Social Worker, Licensed Master Social Worker preferred
  • Experience as a school social worker or as a social worker in a non-school setting preferred.
  • Clinical experience preferred.
  • Willingness to work with special needs students with challenging, sometimes aggressive behaviors.
  • Ability to work effectively with the parents / guardians of special needs students
  • Ability to effectively advocate for students in your care
  • Ability to communicate effectively with staff
  • Willingness to serve students beyond the confines of a regular work day
  • Willingness to visits parents / guardians beyond the geography of the residence
  • Ability to maintain appropriate communication through phone contacts with parents.
  • Willingness to participate in Medical visits.
  • Ability to develop and coordinate referral & support services for students and families.
  • Ability to process intake paperwork.
  • Ability to gather and maintain pertinent family and student information.
  • Ability to develop and maintain case records.
  • Willingness to participate in direct student milieu treatment.
  • Ability to develop relationships with students, families, staff, and outside social service personnel.
  • Evening, weekend, and holiday availability.
  • Driver’s license preferred.
