Are you a Nurse who also has an administrative and business mindset? Are you feeling burned out from the demanding hospital setting? In this Nurse Care Manager role, you will have the opportunity to utilize your clinical expertise as well as your interpersonal and customer service skills. Responsibilities include actively building strong relationships with current and new clients along with partnering with our internal team to grow Assisting Hands home care and care for those living in our communities. You will partner with various departments to initiate high-quality care for the clients.
As a Nurse Care Manager, you will:
· Expand relationships with referral sources and partner with the medical and long-term care community and speak at networking events.
· Conduct initial intakes and ongoing assessments and patient/caregiver education about our services. This will require driving from client to client.
· Supervise PRN/part-time/full-time LPNs and co-supervise CNAs and HHAs.
· Utilize strong written and verbal communication and customer service skills to oversee and form relationships with clients, their family, key partners, and referral sources (case managers, social workers).
· Willing to be flexible where you may need to respond to situations on the weekends (emergencies).
· Oversee the start of care for clients and introduce new caregiver to client. Work in a rapidly changing environment requiring multi-tasking, critical thinking and creative problem solving.
· Use excellent organizational tools requiring technical skills.

Assisting Hands seeks those special LPNs who not only possess strong clinical expertise with innovative ideas but have the kind of deep compassion and sensitivity it takes to treat people in their homes.
· Participate in the development of annual and 3-year strategic plans and related training programs.
· Assist in the identification and development of volume growth plans as well as the implementing of identified initiatives.
· Conduct chart audits. Overseeing the non-skilled record for each client to whom services are provided.
· Initiating the care plan, revising as necessary, and monitoring the appropriateness of services for the client.
· Co-supervising, assessing, and training CNA and HHA providing care. As indicated, completing supervisory oversight of company’s personnel, and completing related documentation.
· Planning for the discharge of clients from service and updating hospital hold status.
· Conduct client satisfaction surveys over the phone for quality assurance.
· Must be willing to obtain a Case Management Certification (company reimbursed).
· Advocating for clients and coordinating ancillary services and healthcare needs that would aid them to improve and maintain their quality of life.
· Assess CNA and HHA on practical competency.
· Maintain a caseload as assigned by their manager.
· Other duties as assigned

Qualifications:
Preferred Bilingual (English and Spanish).
Preferred Associates or B.S. degree in nursing.
Must be willing to travel. This position requires driving from client to client. A typical assessment day includes 3-4 homes. We normally schedule assessments for clients within the same area to facilitate driving. Mileage reimbursement. A company car might be provided.
Assessments are not assigned everyday as you will have "administrative" days during the week.
Nurse (LPN) with current license in state of Florida Minimum two-year clinical nursing
Current CPR certification
Valid and current driver’s license and auto insurance
Background Screening (Level II) clearance
Screened for TB and pass a physical.
