Physical Therapy Care Manager – Home Health
Summary:
This position involves managing and directing physical therapy services for home health patients, including evaluation, care planning, therapy execution, and patient education. The role emphasizes optimizing patient outcomes, collaborating with caregivers, and ensuring compliance with home health standards.
Schedule: Full-time, Monday to Friday 8:00 AM – 5:00 PM, with occasional weekend on-call responsibilities.
Responsibilities:
- Manage a patient caseload requiring physical therapy as the primary service.
- Perform comprehensive assessments, documenting functional abilities, ADLs, and mobility.
- Develop patient-specific care plans with measurable, achievable goals.
- Provide education to patients and families regarding home exercises and safety measures.
- Monitor patient progress and revise care plans as needed.
- Communicate with physicians and interdisciplinary team members regarding patient status.
- Participate in quality improvement initiatives.
Qualifications:
- Graduate of an accredited physical therapy program.
- Current PT license in practicing state.
- CPR/BLS certification.
- Minimum 1 year of clinical experience.
- Reliable transportation and valid driver’s license.
Preferred Qualifications:
- Doctorate in Physical Therapy.
- Home health care experience.
- Certifications in Home Health Care Management or OASIS Specialist-Clinical.
Skills and Abilities:
- Strong clinical assessment and evaluation skills.
- Effective communication, time management, and documentation skills.
- Ability to work independently and within an interdisciplinary team.
Physical Requirements:
- Capable of assisting patients with mobility and therapy exercises.
- Ability to lift, bend, reach, and walk for prolonged periods.
Environment:
- Patient homes with potential exposure to allergens and bodily fluids.
