Physical Therapy Care Manager – Home Health
Summary:
The Home Health Physical Therapy Care Manager provides and directs comprehensive physical therapy services for patients in their homes. This includes assessment, care planning, therapy implementation, patient education, and outcome evaluation. The position requires excellent clinical judgment, scheduling efficiency, and collaboration with interdisciplinary teams to ensure high-quality care.
Schedule: Full-time, Monday to Friday 8:00 AM – 5:00 PM, with occasional weekend on-call responsibilities.
Responsibilities:
- Manage a caseload of home health patients requiring physical therapy as the primary intervention.
- Conduct initial and ongoing assessments of functional abilities and ADLs.
- Develop and implement individualized patient care plans in consultation with physicians and caregivers.
- Provide therapy treatments and monitor patient progress toward goals.
- Educate patients and families on proper care techniques and safety measures.
- Document all assessments, interventions, and outcomes accurately in the clinical record.
- Reassess patients at least every 30 days and adjust care plans accordingly.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Identify performance improvement initiatives and implement quality care programs.
Qualifications:
- Accredited degree in Physical Therapy.
- Active PT license in the practicing state.
- Current CPR/BLS certification.
- Minimum 1 year of clinical experience in physical therapy.
- Reliable transportation and valid driver’s license.
Preferred Qualifications:
- Doctorate in Physical Therapy.
- Experience in home health or as a care manager.
- Certifications in Home Health Care Management or OASIS Specialist.
Skills:
- Strong communication and teaching skills.
- Ability to prioritize tasks and manage multiple patients effectively.
- Attention to detail in clinical assessments and documentation.
- Problem-solving and critical thinking in home health settings.
Physical Requirements:
- Ability to move equipment, assist patients, and perform physical therapy safely.
- Prolonged standing, walking, bending, reaching, and lifting as required.
Work Environment:
- Home-based patient care with exposure to clinical conditions and infection control measures.
