Home Health Physical Therapy Care Manager

Summary:
Manages and provides physical therapy care for home health patients. Responsible for assessments, therapy planning and implementation, patient/family education, progress monitoring, and interdisciplinary collaboration. Ensures compliance with home health care standards and quality care outcomes.

Schedule: Full-time, Monday to Friday 8:00 AM – 5:00 PM, on-call weekends 2–3 times per year.

Responsibilities:

• Conduct initial and ongoing patient evaluations.
• Develop individualized, goal-oriented care plans.
• Implement therapy interventions and monitor patient progress.
• Educate patients and caregivers on home therapy techniques.
• Reassess patients every 30 days and update care plans.
• Communicate with physicians and care teams regarding patient condition.
• Document all care, interventions, and outcomes accurately.
• Participate in quality improvement initiatives.

Qualifications:

• Licensed Physical Therapist in practicing state.
• Graduate of accredited PT program.
• CPR/BLS certification.
• Minimum 1 year clinical experience.
• Valid driver’s license and reliable transportation.

Preferred Qualifications:

• Doctorate in Physical Therapy.
• Home health care or care management experience.
• Home Health Care Manager or OASIS Specialist Certification.

Skills:

• Strong clinical, documentation, and organizational skills.
• Excellent communication and patient education abilities.

Physical Requirements:

• Ability to lift, walk, bend, and assist patients.

Environment:

• Home health environments with exposure to clinical and household conditions.
