Position SummaryEvaluates patients for level of care, provides counseling, linkage, and follow-up services. Guides patients and their families from the Emergency evaluation through all levels of care to hospitalization in the State system.Primary Position ResponsibilitiesProvides thorough evaluations to patients, including collateral contacts with family and treatment provider(s), both in the Medical Center and the community.Provides counseling to patients and their families.Collaborates effectively with Emergency Department, Psychiatry, 2 Central, and other HBH/HHS systems staff.Interface with all acute system of care providers (STCF, State Hospital, ICMS, PACT, etc.) and community providers to facilitate appropriate treatment and case management.Provides accurate and timely documentation of all activities.QualificationsMinimum Education:Required:Bachelor’s Degree in Social Work, Psychology, or related fieldMinimum Years of Experience (Amount, Type and Variation):Required:2 years clinical experiencePreferred:Experience in a crisis settingLicense, Registry or Certification:Required:State of NJ Screening certification (within 6 months of hire)Valid Drivers LicensePreferred:NoneKnowledge, Skills and/or Abilities:Required:NonePreferred:Familiar with State of New Jersey Psychiatric and Commitment laws and process.
