Job Title: Physical Therapy Care Manager – Home Health
Schedule: Full-Time, Monday–Friday, 8:00 AM – 5:00 PM (with limited on-call rotation, approx. 2–3 weekends per year)
Location: Service area includes multiple counties in Missouri and Kansas

Job Summary

We are seeking an experienced Home Health Physical Therapy Care Manager to coordinate and provide physical therapy services for home health patients. This role involves comprehensive assessment, care planning, implementation, evaluation, and ongoing reassessment to ensure quality care. The Care Manager also educates patients and caregivers, collaborates with interdisciplinary teams, and promotes patient independence and optimal outcomes.

Key Responsibilities

• Manage a caseload of home health patients requiring physical therapy as the primary service.
• Perform comprehensive evaluations, including functional assessments of daily living activities.
• Develop individualized plans of care in collaboration with patients, families, and physicians.
• Establish realistic, measurable goals to improve or stabilize medical conditions and functional abilities.
• Provide therapy services, reassess patient progress every 30 days, and update plans of care.
• Document findings, care plans, and progress accurately in the clinical record.
• Report significant changes in patient condition to physicians and adjust care accordingly.
• Optimize scheduling to balance patient needs with efficiency and productivity.
• Educate patients, families, and caregivers on therapy plans and safe practices.

Qualifications

Minimum Requirements:

• Licensed Physical Therapist (PT).
• At least 1 year of relevant clinical physical therapy experience.
• Valid driver’s license and current auto insurance.
• Current American Heart Association BLS certification.

Preferred Qualifications:

• Doctorate degree in Physical Therapy.
• 1 year of recent experience in a Medicare-certified home health agency.
• Home Health Care-Manager Certification.
• OASIS Specialist-Clinical (COS-C) certification.
