Lorien Columbia seeks an experienced Social Worker with a background in long term care case management. The ideal candidate must possess critical thinking skills, patience and emotional strength. You must also be driven in pursuing the social work role and developing your clinical skills.

As a Social Worker, you should be comfortable helping patients adapt and must collaborate daily with our nursing units and other departments on behalf of our patients. Strong written / verbal communication skills are necessary and you must enjoy being a part of a team.

Salary: $50,000 - $75,000 / salary (commensurate with experience)

Responsibilities

• Evaluate and monitor patients' psychosocial wellbeing.
• Collaborate with a multidisciplinary team.
• Provide case management to short-term and long-term patients.
• Assist with admissions and discharge planning processes.
• Plan and attend care conference meetings with patients and their support systems.
• Maintain effective communication with a multidisciplinary team, patients, and their support systems.
• Help with completing advance directive documents, if needed/desired.
• Document interactions and efforts made on behalf of patients.
• Track physician certifications of capacity and MOLST documents to ensure compliance with state regulations.
• Excellent communication, organizational and leadership skills

Qualifications for a Social Worker

• Valid Licensed Social Worker
• Excellent listening and communication skills
• 1+ year of healthcare social work and/or case management experience
• Interest in clinical/healthcare social work
