Position Summary
Bridging Life Hospice is seeking a compassionate, dedicated, and licensed Social Worker to join our interdisciplinary hospice care team. This full-time position provides in-home and facility-based support across Baltimore County and Baltimore City. The Social Worker plays a vital role in supporting patients and families through emotional, social, and practical challenges related to terminal illness.

Position Summary
Bridging Life Hospice is seeking a compassionate, dedicated, and licensed Social Worker to join our interdisciplinary hospice care team. The Social Worker plays a vital role in supporting patients and families through emotional, social, and practical challenges related to terminal illness.
Key Responsibilities
- Assess patient and family needs promptly upon admission, with attention to emotional, environmental, and financial concerns.
- Collaborate with physicians, nurses, and other team members to develop individualized care plans.
- Identify and refer patients to appropriate community resources based on assessed needs.
- Support patients and families in understanding and navigating health decisions, advance directives, placement options, and bereavement.
- Participate in interdisciplinary team meetings and document social work interventions and progress.
- Maintain accurate and timely documentation in accordance with professional, regulatory, and internal standards.
- Monitor and evaluate patient condition and progress, communicating changes to the care team.
- Serve as a key resource in complex situations involving substance use, abuse/neglect, ineffective coping, and end-of-life decision-making.
- Uphold patient confidentiality and promote a team-based, ethical approach to care.
- Engage in ongoing professional development, licensure maintenance, and continuing education.
- Must hold an active LCSW-C, LCSW, or LMSW license endorsed in the state of Maryland.
- Minimum of one year of experience in healthcare or hospice preferred.
- Strong interpersonal, communication, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Reliable transportation required for travel between homes and facilities.
