Role and Responsibility:

• Complete admission assessments and create discharge plans for: (Home care patients, Subacute and acute rehab patients, Homeless patients, Complex and difficult discharge cases)
• May work in one assigned unit or rotate across various units.
• Attend daily discharge planning meetings and collaborate with interdisciplinary staff.
• Communicate frequently with patients and families regarding discharge plans.
• Not a therapy role – focused on care management and discharge coordination.

Requirement:

• Must have 3+ years of hospital discharge planning experience (adult, pediatric, and maternal health patients).
• BLS (Basic Life support), NRP , TNCC
• Must Have MSW license
