Are you a Medical Social Worker looking for some extra income while working on a flexible schedule?

Here at Creek View Home Health & Hospice, we are looking to immediately hire MSWs. This position is per diem (paid on a "per visit" basis). A visit may range between 45mins - 1 hour per Medicare regulations.

Visits involve providing skilled patient care to the patient's home. It is a low pressure environment that allows you to focus fully on the care of the patient.

Basic requirements for this assignment:

• Active Medical Social Worker license in the state of Arizona.
• A least 1 year of home health and hospice experience.
• A valid and up to date CPR certification.
• A reliable mode of transportation. This position requires you to drive to the patient’s home.
