REQUIREMENT: NURSING HOME EXPERIENCE!

Job description: Immediate opening/Nursing Home experience a must. Come Grow With US! SW/Case Manager for a 280 bed skilled nursing facility. Knowledge/experience in Medicare/Medicaid, MLTC, HMO Case Management, Discharge Planning. BSW/MSW. Wage based on experience.

Job Type: TEMPORARY Full-time to possible permanent Full time

Pay: $65,000.00 - $70,000.00 per year

Benefits: When permanent

Schedule:

  • 8 hour shift

  • Monday to Friday
