About Job

Actors Fund Home, an assisted living, skilled nursing, accredited dementia care and sub-acute rehabilitation center located in Englewood, NJ, and a recipient of a CMS Five Star Rating seeks a Social Worker to assist in planning, organizing, developing, and implementing The Home’s Social Services Department to meet the psychosocial needs of the residents in accordance with the mission of The Actors Fund and all pertinent regulations.


RESPONSIBILITIES

The Social Worker will be responsible for counseling, support, and connection to resources for residents and their families; and coordination with staff to enhance quality of life for residents in the facility.

Some responsibilities the social worker will have are:

  • Assess the individual social and emotional needs of each resident and develop a plan of care to meet identified goals.
  • Conduct initial, quarterly, and annual assessments of residents (including MoCA, MMSE and other screening tools).
  • Provide counseling and support; information, psychoeducation, and referrals for residents and their families.
  • Conduct care planning and discharge planning.
  • Maintain all electronic charting documentation and MDS as required by regulations for both Nursing Home and Assisted Living.
  • Participate in QA.
  • Play a leadership role in Comfort Matters dementia care and education program, including scheduling and facilitating meetings; working with interdisciplinary team to encourage staff empowerment and continuing education; working with management to assist in preparing the team for accreditation.

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree required, Masters required in the Social Work profession.
  • Current New Jersey state license as appropriate.
  • Excellent written, verbal, and interpersonal communication skills.
  • Strong assessment and advocacy skills and the ability to engage a wide range of presentations.

ADDITIONAL REQUIREMENTS

  • Hours will incorporate rotating weekend coverage.
  • May be asked to cover for Director of Social Services.
  • Experience with Geriatric social work.

This is a full-time salaried position ($70,000-$75,000) based in the Actors Fund Home in Englewood, NJ.
