Position Summary

Breeze Hospice is hiring a compassionate LCSW or MSW Hospice Case Manager to join our care team serving patients throughout the St. Louis area. This role is ideal for a licensed social worker who values meaningful relationships with patients and families, the autonomy of in-the-field work, and the support of a collaborative interdisciplinary team.

As a Hospice Social Worker, you’ll play a vital role in supporting patients and families through emotional, psychosocial, and practical challenges related to end-of-life care. Your expertise will help ensure patients experience dignity, comfort, and compassionate support throughout their hospice journey.

Position Details:

  • Full Time
  • Home-based visits within a designated service area
  • No weekends required
  • Manageable caseload to ensure quality, focused patient care
  • Support from RNs, aides, and administrative staff to streamline your workflow

Responsibilities:

· Provide psychosocial assessments and individualized care planning

· Offer emotional and practical support to patients and their loved ones

· Assist families with advance care planning, community resources, and grief counseling

· Collaborate with the interdisciplinary team, including RNs, physicians, chaplains, and CNAs

· Serve as an advocate and liaison between patients, families, and community services

· Maintain accurate and timely documentation in accordance with regulatory and organizational standards

Qualifications:

· Master’s degree in social work (MSW) from an accredited program

· Current Missouri LMSW or LCSW license required

· One year of hospice, palliative care, or healthcare experience preferred

· Strong communication, counseling, and problem-solving skills

· Reliable transportation and a valid driver’s license
