Schedule: Full-Time, 40 hours/week, Mon–Fri 8 a.m.–5 p.m., with flexibility evenings/weekends as needed

Dress Code: Business Casual (Jeans/t-shirts only on Friday with approval)

Summary of Position
The Permanent Supportive Housing (PSH) Case Manager provides case management for clients who are housed, using a trauma-informed approach. The goal is to help participants maintain stable housing and create a supportive community. This role includes conducting home visits, collaborating with property managers, linking participants to other providers, and ensuring compliance with program requirements. If a client loses housing, the Case Manager is responsible for identifying alternative options. Documentation in HMIS and other systems is required, along with occasional program coverage and volunteer support.
Essential Duties & Responsibilities
Services (approximately 70% of time):
- Maintain program capacity and support move-in/move-out processes, assisting tenants with documents.
- Provide case management to 12 scattered-site clients including intakes, advocacy, home visits, and referrals.
- Conduct bi-monthly life skills groups and ensure rent collection/receipts are documented.
- Link clients to mental and physical health providers and other supportive services.
- Develop, update, and monitor service plans and document outcomes in a timely manner.
- Complete SAMHSA SOAR Training within the first year if not already trained.
- Obtain HUD Counselor certification as required.
Administrative (approximately 20% of time):
- Update leases and submit rent payment vouchers.
- Input accurate data into HMIS and other required systems.
- Maintain organized client charts with completed forms.
- Attend staff and supervisory meetings as required.
Other Duties (approximately 10% of time):
- Provide program coverage support when needed.
- Engage with and support volunteers.
- Facilitate groups or trainings as identified by the supervisor.
Qualifications
Education & Experience:
- AA in Human Services, Social Studies, or Health (or equivalent experience/education).
- 1–2 years working with individuals experiencing homelessness and/or trauma.
- Skills & Abilities:
- Bilingual (English/Spanish) strongly preferred.
- Proficiency in MS Office, Google Docs, and HMIS.
- Strong communication, teamwork, and problem-solving skills.
Other Requirements:
- Valid CA driver’s license, proof of insurance, and insurable driving record.
- Must have a clear State DOJ and FBI criminal background check.
- **Must be able to pass a pre-employment drug screen.**
- Completion of 40-hour Domestic Violence Counselor/Advocate certification (State of CA).
- Must complete SOAR training (within first year) and HUD Counselor certification as required.
Work Environment
The work environment includes a normal office setting, home visits, and community sites. Noise levels range from quiet to moderately noisy. This position requires flexibility in working conditions, including evenings and weekends as needed.
