DESCRIPTION

Under general supervision, and in addition to performing the full range of operational level duties, responsible for administering social service programs or relevant work. Provides social work services consisting of discharge planning, continuing care services, and clinical social work services, including crisis intervention, mandated reporting, information and referral, and coordination of health and mental health services with community providers.

The social worker is responsible for education and advocacy of patients, families and/or significant others. Participates in related department functions, such as community and program education. Performs all other duties as assigned.

MINIMUM QUALIFICATIONS

  • A Master's degree in social work from an accredited school.

  • At least two (2) years of post-MSW experience.

  • Must possess the knowledge and abilities essential to the successful performance of the duties assigned to the position.

  • Must have proven independent thinking, decision making, problem-solving and prioritizing skills.

  • Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients, and external constituents, both verbally and in writing.

PREFERRED QUALIFICATIONS

  • Licensed Clinical Social Worker (LCSW)
  • Bilingual in both English and Spanish.
  • Epic experience.
  • 2 years of post-graduate clinical experience.
  • 2 years experience working with varied levels of population.
  • 1 year experience working in a hospital acute care setting, Inpatient or Emergency Department.
  • Experience working with high levels of social determinants patient populations, polysubstance abuse/dependence patient populations, unsheltered, and medical social work.

SPECIAL CONDITIONS

  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical.

Pay Transparency Act

Annual Full Pay Range: $107,302 - $107,302 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $51.39 - $51.39

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
