Health Care Coordinator (RN)

This is a part-time position with flexibility and is in service to Congregation of St. Joseph Catholic Sisters in the New Orleans area.

The Health Care Coordinator, who reports to the Clinical Operations Director for the Congregation of St. Joseph, promotes a holistic approach to a healthy lifestyle with a focus on personal responsibility and health maintenance. A critical component to this position is to coordinate care and support for the Sisters, especially during times of illness that may require significant decision making regarding health and well-being.

Essential Functions

Included here is a partial list of essential functions.

  • Consults with Sisters regarding healthcare concerns. Assists Sisters in developing a healthcare plan which may incorporate both alternative health care approaches and traditional medical treatment;
  • Participates in forming, evaluating, formulating and monitoring treatment(s) and service plans as needed and as requested by Sisters;
  • Assists Sisters with managing their documents as needed; to include their POA or other responsible party documents as changes occur with the progression of time.;
  • Assists Sisters in understanding medical information and evaluating feedback from medical providers to ensure services rendered are promoting the highest quality of life for the Sisters;
  • Assists to help coordinate transportation for Sisters to medical appointments and treatments as needed;
  • Pursues a professional working relationship with medical providers to obtain, understand and facilitate Sisters’ care when it is needed;
  • Assists in facilitating communication with other involved interested parties, which may include biological family, other caregivers, friends and/or other Sisters within HIPAA guidelines; and
  • Represents the best interests of Sisters who reside in long term care and memory care units as the clinical overseer/facilitator of their plan of care by attending care plan conferences, maintaining communication with the facility nurse leadership team ongoingly.

KNOWLEDGE, SKILLS & ABILITIES

  • Willingness to learn and an understanding of religious life, faith beliefs and spiritual practices supportive to the life of women religious;
  • Ability to work collaboratively with various personality profiles;
  • Ability to maintain clear, effective, open and honest communication with all involved in the Sisters' care;
  • Ability to prioritize and triage issues of highest importance;
  • Ability to multitask and demonstrate effective time management; and
  • Must have good organization skills with attention to detail.

EDUCATION & EXPERIENCE

  • Nurse License or Social Worker License, or other licensed health care practitioner;
  • Basic medical/clinical knowledge;
  • Experience in working with vowed religious preferred but not required;
  • Previous experience in dealing with older adults with a sensitivity to the aging process;
  • Strong organizational skills; and
  • A valid state driver's license.

WORKING CONDITIONS

  • 35% travel; and
  • Some evening and/or weekend work, when needed.
