Job Summary:

CareLinx by Sharecare is presently employing either a Licensed RN, ORN, LPN, LVN, licensed social worker ("Assessor") to complete a one-time in-home assessment. Solutions for Caregivers is partnering with CareLinx to provide care management benefits to family caregivers and members by deploying RNs or FCAs to complete a one-time in-home assessment (approximately 3-4 hours long).

The assessment you complete will provide essential information back to the Care Manager within the Solutions for Caregivers team. You are the "eyes and ears" of the Care Manager while you are in the member's home. You are using the assessment to gather information, document your observations in the home, make recommendations, and evaluate the safety of the care recipient. Your complete assessment will provide the Solutions for Caregivers team insight to better meet the needs of the care recipient and the caregiver in the home, if present.

Requirements: RN, LPN, LVN, licensed social worker ("assessor") must be currently licensed and COVID vaccinated who is:

• (a) duly qualified and licensed or registered under the laws of the jurisdiction in which Care Management Services are rendered and/or certified by the appropriate certification agency,
• (b) who practices as a partner or employee,
• (c) is identified by profession in the applicable Statement of Work, and
• (d) who meets the following criteria:
• Nurses: Minimum of Registered Nurse (Bachelor of Science Nursing/BSN preferred), Licensed Practical Nurse (LPN) with current state licensure.
• Social Workers: Minimum of master’s degree in social work, with current state licensure or registration.
• Other Field Care Advocates: Minimum of a master’s degree in pertinent core discipline and current certification as care manager (e.g., gerontologists, Case Manager Certified/CMC or Certified Care Manager/CCM). For licensed clinicians (i.e., nurses, social workers)
• A minimum of three years of experience in providing care management services is preferred.

• Thorough documentation of care plan tasks and activities completed in the home
• Timely submission of timesheets via the CareLinx app
• Communication with care advisors as needed
• Ensures that all information about members is maintained as confidential
• Covid-19 vaccinated
