In this role, you'll be more than just a manager—you'll be a trusted ally and advocate for those you serve. Your main focus will be on delivering personalized care management that truly puts the individual at the center of everything we do. Responsibilities include: conducting clinical assessments, crafting individual support plans, monitoring services (in-person) and coordinating care. This position will primarily support individuals in Durham, Orange and Person Counties.
Collaboration will be key in this role, as you work closely with a diverse network of partners, including local and statewide agencies, healthcare professionals, and community organizations. Together, you'll coordinate holistic care that respects the unique needs and preferences of each individual, ensuring they receive the support and resources they need to live their best lives.
Job Type: Full-time
Pay: $45,000.00 - $52,000.00 per year
