Summary

The Community Support Team (CST) Qualified Professional provides intensive, community-based services to adults with severe and persistent mental illness (SPMI), often with co-occurring substance use disorders. The CST QP works as part of a multidisciplinary team to support recovery, reduce hospitalizations, promote independent living, and connect members with community resources.

Key Responsibilities

Direct Services

Minimum 2 years experiance with Adult Mental Health Clients

• Provide face-to-face services in the community (minimum of 80% of time spent in the field).
• Deliver interventions consistent with the NC Medicaid CST service definition (8A Clinical Coverage Policy).
• Assist members with symptom management, daily living skills, relapse prevention, and recovery planning.
• Support members in accessing housing, medical care, employment, transportation, and community resources.
• Participate in crisis response and stabilization, coordinating with emergency services when needed.

Care Coordination

• Collaborate with team members (Team Lead, Peer Support Specialist, Nurse, and others) to provide integrated care.
• Facilitate referrals and linkage to higher or lower levels of care when clinically indicated.
• Advocate for client needs with community partners (hospitals, DSS, housing agencies, probation officers, etc.).
• Engage natural supports and families as appropriate in treatment planning.

Documentation & Compliance

• Complete Person-Centered Plans (PCPs), service notes, and updates in compliance with Medicaid and MCO requirements.
• Track authorizations, service utilization, and ensure documentation supports medical necessity.
• Maintain accurate, timely, and audit-ready records in the Electronic Health Record (EHR).
• Participate in Clinical Quality Reviews (CQRs) and internal quality assurance audits.

Team Participation

• Attend and actively contribute to weekly CST team meetings.
• Provide input on treatment planning, goal development, and discharge planning.
• Maintain open communication with supervisors and MCO care coordinators.

Job Types: Full-time, Part-time

Pay: $22.48 - $27.07 per hour

Expected hours: No less than 35 per week
