Job Title: Social Worker

Job Summary:
We are seeking a compassionate and resourceful Social Worker to support individuals and families in addressing emotional, social, and life challenges. The Social Worker will assess client needs, develop care plans, connect clients with resources, and provide counseling and advocacy to promote well-being and self-sufficiency.

Key Responsibilities:

  • Conduct assessments to identify client needs, strengths, and goals.
  • Develop and implement individualized care or intervention plans.
  • Provide counseling, crisis intervention, and emotional support.
  • Connect clients with community resources, services, and programs.
  • Advocate for client rights and access to services.
  • Collaborate with healthcare providers, educators, and community organizations.
  • Maintain accurate and confidential case records.
  • Monitor and evaluate client progress, adjusting plans as needed.
  • Participate in team meetings, training, and professional development.
  • Adhere to ethical and legal standards for social work practice.

Qualifications:

  • Bachelor’s or Master’s degree in Social Work (MSW preferred).
  • State licensure or certification as required.
  • Experience in counseling, case management, or related social services field preferred.
  • Strong interpersonal, problem-solving, and organizational skills.
  • Ability to work with individuals from diverse backgrounds.

Work Environment:

  • May work in schools, hospitals, clinics, government agencies, or community settings.
  • Occasional evening or weekend work may be required depending on client needs.
