We are hiring a Care Manager for our Decatur Office. This position is available immediately.

The role of the Care Manager (CM) is to provide the day-to-day coordination and scheduling of quality and qualified field employees with clients. The CM is responsible to maintain current client services and intake of new clients to agency. CM is held accountable to communicate and update all client care information into agency EMR system to ensure clients’ Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) needs are met by all members of the interdisciplinary team.

This position will be based out of the Decatur branch and covers the surrounding counties.

We have a stable client census with consistent assignments and hours, offering you the ability to work with clients and their caregivers long-term.

Care Manager

Responsibilities:

  • Participate in all general office administrative operations including on-call responsibilities to ensure business continuity.
  • Manage intake coordination tasks from all referral sources while continuing to seek new referral opportunities to support branch operational growth goals.
  • Provide the overall supervision of field caregivers and perform annual performance reviews to promote professional growth goals.
  • Learn and familiarize self with available workforce and their respective skill levels to effectively coordinate caregiver scheduled based on client care needs and diagnosis.
  • Manage client/contract satisfaction with the highest level of customer service standards, communication, and documentation.
  • Ensure all forms of communication with clients, families, health plan providers, and caregivers are documented timely to promote continuity of care across all interdepartmental functions.
  • Responsible for generating client care plan assessments and system updates.
  • Conduct audit review of caregiver’s daily documentation logs to ensure compliance with client’s ADL and IADL needs.
  • Build relationships with clients to encourage compliance with care plans and client safety.
  • Orientate and supervise field caregivers to client care plans and provide remedial training and mentoring as needed.
  • Assess caregivers’ compliance and understanding to client care plans on a quarterly basis or as needed based on client’s level of care and changes in conditions in between quarterly visits.
  • Work with Recruitment and Compliance departments to communicate recruitment needs and ensure active caregivers are in compliance with state and federal annual health and training requirements.
  • Manage time and attendance of caregivers and ensure daily compliance with Electronic Visit Verification (EVV) requirements for timely payroll and billing processing.
  • Run reports and utilize data from EMR and Electronic File Storage System to manage KPIs.
  • Follow Agency Policies and Procedures
  • Ensure all HIPAA regulations are adhered to.
  • All other duties as assigned.

Licensure/Certification Requirements:

  • Valid driver’s license, and auto insurance coverage

Other Skills/Requirements:

  • Minimum of two years related experience required. Working knowledge of commonly used medical concepts, practices, and procedures preferred.
  • Essential Technical Skills: Working knowledge of Word, Excel, Windows, Email, typing of 35 wpm and accurate data entry skills
  • A specific understanding of geographical/cultural requirements of branch office is preferred.
  • Must be customer service oriented and prepared to liaise with clients, their families and other care providers.
  • Ability to concentrate with frequent interruption, handle pressure of deadline, good judgement, ability to follow procedures, ability to work independently.
  • Effective communication skills, both verbal and written
  • Willingness to embrace technology/utilize computerized systems
  • Bilingual, preferred, but not required
