- Implementation or delegation of responsibility for twenty-four hours nursing service and home health aide services.
- Coordination of services with the patient, family, and others involved in the plan of care.
- Regular evaluation of patient's progress, prompt action when any change in the patient's condition is noted or reported, and termination of care when goals of management are attained.
- Responsible for the observation, assessment nursing diagnosis, counsel, care and health teaching of the sick, injured and infirmed, and the maintenance of the health and prevention of the illness of others.
- Completes initial nursing assessment within 48 hours of the client's admission and prepares care plan.
- Explains all procedures, treatments, medications, and client's plan of care to client and family, as appropriate.
- Charts progress notes, reviews client status and care provided. Provide progress notes to the patient's health care provider about the patients under care when the patient's conditions change to there is a deviation from the plan of care or at least every sixty days.
- Documents and communicates client responses and the effectiveness of nursing/medical interventions in the client's clinical record.
- Directs the activities of the clinical care to LPNs, CNAs, and other ancillary staff. Prepares written instructions and supervises the staff in the home, in accordance with policy, and state and federal regulations.
- Administers medications and treatments as prescribed by the client's physician.
- Act as the coordinator of the health care team to maintain continuum of care with all disciplines, including contracted services.
- Demonstrates a thorough knowledge of and practices within the Infection Control Policies and Procedures established.
- Assists with restorative services including ambulation and related therapies.
- Participates in the on-call rotation.
