Overview:
Enoved & Co. is seeking a dedicated and compassionate PRP Counselor/Case Manager to join our team. This position offers flexible hours and the opportunity to make a positive impact in the lives of individuals accessing our Psychiatric Rehabilitation Program (PRP). Responsibilities:
- Provide case management services to individuals enrolled in our PRP, including assessment, treatment planning, and coordination of services.
- Facilitate skill-building groups and individual sessions to support clients in achieving their goals related to independent living, social skills, and community integration.
- Collaborate with clients, families, and treatment teams to develop and implement individualized rehabilitation plans.
- Assist clients in accessing community resources and support services to enhance their overall well-being and quality of life.
- Maintain accurate and up-to-date documentation of client progress, treatment interventions, and service coordination activities.
- Conduct comprehensive assessments of clients' needs, strengths, and goals
- Coordinate and monitor services provided to clients, ensuring they are receiving appropriate care and support
- Advocate for clients' rights and access to resources
- Maintain accurate and up-to-date client records, documenting progress and outcomes
- Stay informed about current research, best practices, and relevant regulations in the field of case management
Skills:
- Bachelor’s Degree from an accredited university or college (or 30 credit hours toward degree) in a health-related field preferred.
- Alternatively, an Associates Degree in a health-related field or one (1) year of work experience in a supervised mental health setting.
- Strong interpersonal and communication skills, with the ability to establish rapport and build positive relationships with clients, families, and stakeholders.
- Valid driver's license and clean driving record required.
- Must be able to pass a background check.
- Excellent communication skills, both verbal and written
- Ability to work collaboratively as part of a multidisciplinary team
- Strong organizational skills with the ability to prioritize tasks effectively
