Position Summary:

The Center is seeking a compassionate and culturally competent HRSN Care Manager to support our role as a Community-Based Organization (CBO) under the New York Health Equity Reform 1115 Medicaid Waiver in partnership with Healthy Alliance. This position will play a pivotal role in addressing unmet health-related social needs (HRSN) by providing direct screening, navigation, and enhanced case management services to refugee and immigrant community members.

The HRSN Care Manager will offer tailored support to individuals who often experience fragmented care, language barriers, and limited knowledge of available services. This role will ensure a coordinated and client-centered approach to health equity, grounded in cultural awareness and community trust.

Key Responsibilities:

  • Assess and complete HRSN assessment to determine eligibility for enrollment
  • Complete intake and enroll refugee and immigrant individuals the UniteUs platform.
  • Provide culturally appropriate navigation support to connect clients to health, housing, food, employment, and other social supports.
  • Deliver enhanced case management services to individuals with complex or ongoing needs.
  • Develop and implement individualized service plans based on identified needs.
  • Maintain regular follow-up to ensure connection to services, monitor outcomes, and adjust plans as necessary.

Coordination & Communication

  • Collaborate with internal Center staff across programs to identify clients who may benefit from the HRSN program.
  • Accept and manage referrals from external organizations, including other social service providers and health systems.
  • Maintain strong communication with healthcare and community partners to ensure seamless, coordinated care.

Data Management & Reporting

  • Accurately document assessments, referrals, service plans, and follow-ups in the UniteUscase management system.
  • Meet monthly service targets (e.g., screenings, case management cases) as outlined in the implementation plan.
  • Provide monthly reports and qualitative updates on service impact and client outcomes.

Required Qualifications:

  • Bachelor’s degree in social work, public health, human services, or a related field. Equivalent experience may be considered.
  • At least 2 years of experience in case management, community health navigation, or similar direct service role.
  • Demonstrated experience working with refugee and immigrant populations.
  • Fluency in English and proficiency in a language spoken by refugee and immigrant communities (e.g., Arabic, Karen, Burmese, Spanish) is strongly preferred.
  • Familiarity with social determinants of health and community-based health strategies.
  • Strong organizational and interpersonal skills with attention to cultural sensitivity.
  • Competency in Microsoft Office, data entry, and case management software.

Preferred Qualifications:

  • Experience working within Medicaid-funded or care coordination programs.
  • Knowledge of local and regional human service and healthcare systems.
  • Lived experience as a member of a refugee or immigrant community is a plus.
