Job Overview – Care Coordinator (Case Management):
Compensation: $25/hour
Location: Brooklyn, NY
Schedule: Full-Time (35 hours per week)
Our client is hiring a Care Coordinator (Case Management) in Brooklyn, NY, to provide case management and client advocacy for individuals with HIV/AIDS and complex social needs. You will conduct intakes, develop service plans, connect clients to healthcare, housing, and community resources, and support long-term stability through ongoing advocacy and progress monitoring.
Responsibilities as the Care Coordinator (Case Management):
- Client Intake & Assessment: Conduct intakes, eligibility reviews, and reassessments to determine client needs and services.
- Case Management: Manage a caseload of clients, develop service plans, and monitor progress toward goals.
- Client Advocacy & Support: Serve as a liaison with healthcare, housing, legal, and social service providers while escorting clients to essential appointments as needed.
- Progress Monitoring & Documentation: Track client outcomes through visits and calls, maintain accurate service plans and progress notes, and prepare required reports.
- Community Engagement: Facilitate support groups, coordinate events, and assist with outreach, benefits applications, and program promotion.
Qualifications for the Care Coordinator (Case Management):
- Education: Associate's or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
- Experience: 2+ years in case management, social services, or community health, with experience supporting HIV/AIDS populations strongly preferred.
- Technical Skills: Proficiency in Microsoft Office Suite and experience with electronic case management or client database systems.
- Industry Knowledge: Understanding of HIV/AIDS, Medicaid, entitlements, and community-based health and social services.
- Skills & Attributes: Strong interpersonal, communication, and problem-solving skills with the ability to advocate for clients and build trust.
- Language Skills: Bilingual in English and Spanish or Haitian Creole preferred.
- Additional Requirements: Ability to travel locally to client homes and appointments as needed.
